How to use the Teacher Feedback Form and Teacher Reflection Form

The first step is to highlight and copy the text

Go to Edit on the menu bar and choose Select All

Go to Edit on the menu bar and choose Copy

Open a new MS Word document

Go to Edit on the menu bar and choose Paste

Save as an MS Word file

Edit the sample feedback form to meet your needs. What you ask, and how you ask it, shapes the respondents' reflections.

Save the new document-your congregation's feedback form .

Use the form in print or you can email your feedback form as an attachment. You might provide the following instructions:
