Sample Teacher Evaluation
Jennifer Morley, DRE
Unitarian Universalist Fellowship of Athens, Inc., GA

Teacher's Evaluation for the year 2000-2001 (Please use back of paper for more space, if needed!) Name_____________________________ Class___________________
Curriculum used____________________________

  1. About the curriculum used this year:
    • Was it clear?
    • Did the kids enjoy the discussions?
    • Did they enjoy the activities?
    • Were the lessons the right length, or was there too much to fit into the hour?
    • Would you recommend using this curriculum in the future? Any other comments?
    • How did you feel about the special lesson plans that the DRE prepared for times like holidays and Environmental Action Weeks? (using the above criteria)

  2. About being a teacher:
    • Was there enough support from the DRE, the RE Committee, the other teachers on your team, and the congregation as a whole? Please describe from where you felt good support and from where you would have liked more support.
    • Did you feel that the parents of the children in your class were involved enough? Do you have ideas about ways to get parents more involved?
    • What did you like best about teaching?
    • What would you want to be different about your experience if you were to teach again?
    • Any other comments, questions, or suggestions?

  3. If you decide to teach again next year, you'll be given first choice of which class to teach and of scheduling.
    • Would you like to teach again next year?
    • Which class do you want to teach?
    • Do you prefer the continuity of teaching two or more weeks at a time, or is your schedule such that you'd prefer to stay with the once-a-month rotation?

  4. About the classroom space, supplies, etc.:
    • Did you feel that the space was adequate for the class's needs?
    • Were there adequate supplies in the cabinets? How about the weekly supplies set out for each specific lesson plan-were they adequate? What you were expecting that wasn't there?
    • Any other comments?

Thank you for your honest answers -- they'll help to improve the teaching experience for all in the years to come!
