Applicatoin Form Congregational Social Justice Empowerment Program

We, the leaders of the________________________________________     congregation in________________________________________     want to schedule a Social Justice Empowerment Workshop with the UUA's Office for Congregational Justice Making. We have discussed this possibility with the members of the governing board of the congregation, the leaders of the congregation’s social justice programs, and the minister or ministers.  We agree to:

1. Fill out the enclosed Congregational Social Justice Evaluation Form.

2. Publicize the event.

3. Recruit participants.

4. Have at least one half of the members of the Board attend.

5. The minister(s) is willing to attend and participate.

6. Do an evaluation after the training event is over.
