How Your Premiums Are Determined

Premiums are based on:

• The choice of plan—Standard PPO or High Deductible PPO
• The age of the employee
• The ZIP code in which the employee lives.
• The category of coverage selected: employee only, employee plus spouse or partner, employee plus child(ren), family.
