How Do I Take Advantage of this UUA Health Plan Offering?

• Read this document carefully and review the benefit details to see if either the PPO or High Deductible Plan is right for you.
   
• Check the Highmark Blue Cross Blue Shield website to see which of your healthcare providers and others you might like to select are in the network. Go to www.highmarkbcbs.com , click on the "Find a Provider" link on the right, and pick a geographic region to search. Click on the "Guest" tab and choose PPO as the Product Type. While you are on the Highmark page, click on "Member Site Demo" for a tour of all the support features the plan offers, from ordering ID cards to checking the status of a claim to resources for managing your health. You may also call Highmark Blue Cross at (800) 810-2583; an associate will do the lookup for you.
   
• Review the information on this website for information about your individual premiums.
   
• Complete the Enrollment Application. Make sure you include the name, phone number, and email address of the church officer in the boxes on the bottom of the form.
   
• If you prefer, you may download the application, print it out, complete it manually, and sign it. Mail to the UUA Health Plan Office, 25 Beacon Street, Boston, MA 02108.
   
• To request an Enrollment Application to be sent by mail or email, send an email to healthinsurance@uua.org .
